PAR Q (Physical Activity Readiness)

A PAR+Q stands for, Physical Activity Readiness Questionnaire. If you're unsure if you should exercise, fill out this form and send it to me. It will let us know if you're ready for exercise. As always check with your Primary Care Doctor before starting any exercise regime.
